Send SMD three photos of yourself, one facing forward (not smiling) and one of each side profile.

 

Our team will choose one patient to be our live injection model for the Kybella Premiere Event on  Monday, April 11th 2016 at 5:30pm and the treatment(s) will be free!

 

Photos must be received by April 4, 2016.
