Mohs surgery is unlike many other surgical procedures because treatment and microscopic analysis occur on the same day. While the length of the visit can vary, understanding the process ahead of time can help you feel more comfortable and prepared for the day of surgery.
Arrival & Preparation
Mohs surgery is performed under local anesthesia. You’ll be awake during the procedure, but the area being treated will be fully numbed using local anesthesia. You can eat breakfast and take your usual medications unless instructed otherwise.
Because Mohs surgery is performed in stages and appointment length can vary, we encourage patients not to schedule other commitments on the day of surgery and to plan to be with us for most of the day. This flexibility allows our team to provide the time and care needed to complete treatment thoroughly and safely.

The Surgical Process
After the visible portion of the skin cancer is removed, the tissue is carefully mapped, processed, and examined under a microscope on-site. If cancer cells remain, their precise location is identified and only the affected area is treated further. This process is repeated until clear margins are achieved.

Waiting Between Stages
While tissue is being examined, you’ll rest comfortably in a private area. Our team will keep you updated throughout the process and are always available to answer questions.
Waiting periods are a normal part of Mohs surgery and are what allow for the precise, real-time margin evaluation that makes the procedure so effective.
Reconstruction & Repair
Once the cancer has been fully removed, reconstruction is planned and performed, most often the same day. The type of reconstruction depends on the location and size of the defect. Some defects are allowed to heal on their own, through a process known as second intention healing. Most commonly, the defect is sutured closed in a straight line (primary closure) or by sliding adjacent skin into the area (local tissue flap). Occasionally a skin graft is needed.
Reconstruction is handled thoughtfully and with attention to both healing and appearance, and you’ll be given clear instructions for aftercare before you leave.

After Surgery
Mohs surgery at Southern Marin Dermatology is performed in an outpatient setting. Once your procedure and any necessary reconstruction is complete, you will go home the same day.
Most patients are able to drive themselves home following Mohs surgery. However, depending on the location of the surgical site—such as areas near the eye, hands, or other functionally sensitive regions—a driver may be recommended for comfort and safety. Our team will review this with you in advance so you can plan accordingly.
Before you leave, you’ll receive clear, written instructions for wound care, activity guidelines, and follow-up. If questions arise after surgery, our clinical team remains available to support you throughout your recovery
